Let's design the questionnaire

Let's go back to the issue of designing a questionnaire.
Let's first ask ourselves some questions:
1. what is the aim of the questionnaire?
2. what are we going to ask SS?
3. what format (open/closed), length and arrangement should the questionnaire have?
4. what format should responses have?
5. how should it be administered and evaluated?
